Center for Elders' Independence

Center for Elders' Independence

The Center for Elders’ Independence helps you live at home, instead of a nursing home. PACE is a health plan designed exclusively for seniors. If you are age 55 and older, dealing with multiple health challenges and need assistance in your daily routine, CEI might be just the help you need.

The mission of the Center for Elders' Independence (CEI) is to promote independence, dignity, and an improved quality of life for older adults with complex health needs in Alameda County and Contra Costa Counties. With a commitment to fostering an inclusive society where older adults lead independent and fulfilling lives, CEI empowers seniors to age gracefully and thrive within their own homes, surrounded by the support and care they deserve. CEI utilizes the Program of All-Inclusive Care for the Elderly (PACE) model of care, utilizing an interdisciplinary care team (IDT) to offer individualized comprehensive medical & social services, and long-term supports to older adults. CEI currently serves over 1100 participants from 5 PACE Centers in Oakland, San Leandro, Berkeley, and Concord.
